What To Bring For ID

We are required by the Cass County Health Department to keep a photo ID with a birthdate on file for every client that has a procedure(s) performed at Dead RockStar.

Clients age 18 and over...

We accept the following forms of (valid) ID:

*Driver's license
*Operator's permit
*Military ID
*Tribal ID
*Passport
*Identification card

Minor Clients age 16 and 17...
 
We are required to keep a photo ID (with a birthdate) from the client as well as the client's parent/guardian's photo ID (with a birthdate) on file. 

We CANNOT and WILL NOT perform any procedures on any minor without proper identifiaction from all necessary parties.

We will NOT perform any surface/dermal anchors, surface piercings, nipple piercings and/or genital piercings on any person under 18 years of age. We also will not tattoo any area on a minor that a swimming suit does not cover. No Exceptions!!!

Parents:
Only a minor client's birth parent(s) may sign consent for the minor to have a procedure performed at Dead RockStar. We only require one parent to sign for consent of a minor. If the client's photo ID has a different last name and/or address than the parent who is signing consent; we require a copy of the client's birth certificate accompanied by the photo ID.

Guardians:
A minor client's guardian may sign consent for a procedure provided that we have a photo ID (which include birthdates) from the client and their guardian. We also require copies of the court documents that prove guardianship.

We accept the following forms of (valid) ID for minors:

*Driver's license
*Operator's permit
*Military ID
*Tribal ID
*Passport
*Identifiaction card
*Student ID accompanied by the client's birth certificate
*A recent yearbook (within the last two years) that includes the client's photo accompanied by the  client's birth certificate
